Here is the updated code, it’s the entire fluid-video-embeds.php file. The additions I made are highlighted in Blue.  Just copy and paste this code replacing everything in the original fluid-video-embeds.php and hit save.  I can’t guarantee that this will work on every single YouTube url, but it seems to work for the ones I checked.

The only thing that could be changed to work better would be the preg_match items on lines 613-628, where I have them grabbing everything from list= (to the end).. whereas if list= is not the last item, it will grab things that go beyond the list=xxxx?extra-stuff… but it seems to work fine as it is.
